    I, the Eucharistic Visitor supervisor for my congregation, request a Eucharistic Visitor license for the following individual.
  • Please indicate the date this individual was confirmed or received in the Episcopal Church.
    The certificate should state that the individual has completed the following 6 courses: Introduction & Theological Background, Healthy Boundaries, Inclusion, Organizational Rules & Policies, Pastoral Relationships, Abuse & Neglect
  • I hereby certify that this applicant is baptized, is a confirmed or received adult communicant in good standing, has been appropriately trained per diocesan policy, and is a fit and responsible person to be a Eucharistic Visitor in this congregation.
